1. Acceptance and eligibility
By using the service, you agree to these Terms and acknowledge the Privacy Notice and applicable Notice at Collection. The public cardiovascular assessment is intended for adults. Do not submit another person’s information unless legally authorized.
Customer organizations and workforce users may also be subject to a master services agreement, order form, business associate agreement, data-processing terms, security requirements, and organization policies. Those agreements control if they conflict with these public Terms.
2. Educational and clinical-support limitations
Cardiology Insights provides one adaptive cardiovascular assessment that may combine symptoms, health history, risk factors, optional known health numbers, educational risk information, urgency guidance, practice matching, and operational routing. It does not provide a definitive diagnosis, prescribe treatment, guarantee that a condition is present or absent, or replace evaluation by a qualified healthcare professional.
Results depend on the information entered and on the approved calculation, clinical release, safety rules, and routing configuration in effect at the time. Incomplete, inaccurate, misunderstood, or changing information can affect the result.
3. Emergency care
Do not use this website to obtain emergency care. Call 911 or the applicable emergency number for severe or ongoing chest pressure, severe shortness of breath, fainting, stroke symptoms, sudden severe pain, or other symptoms that may be immediately dangerous.
Emergency safety instructions take priority over assessment ranking, verification, practice selection, payer matching, capacity, and routine scheduling.
4. Assessment, practice selection, and patient access
A neutral visitor may review an educational result without choosing a practice. If eligible, the visitor may search for an available participating practice. Information is transmitted only after the visitor selects a practice and authorizes sharing.
A route, verification code, secure-completion link, task, appointment request, or displayed recommendation is not a confirmed appointment. The participating medical organization makes final clinical, physician, location, payer, referral, scheduling, and treatment decisions.
Use of the platform does not by itself create a physician-patient relationship with Cardiology Insights or a listed physician. Cardiology Insights is not the treating medical practice.
5. Verification and communications
When you authorize patient access, you consent to transactional verification, secure-completion reminders, and contact about the specific request by Cardiology Insights and the selected organization. Standard carrier or data charges may apply. These communications are not consent to unrelated marketing.
Family Heart Check links are generic and contain no originating patient name, result, diagnosis, insurance information, or contact information. Send a family link only in a lawful and respectful manner; each recipient completes an independent assessment and makes independent choices.
6. Authorized workforce accounts
Customer workforce users must protect credentials, use only assigned roles and organization scope, access information only for legitimate duties, follow the participating organization’s privacy and security policies, and immediately report suspected misuse or unauthorized access.
Support View is read-only. Platform administration, clinical release, privacy operations, and cross-tenant functions are limited to specifically authorized roles and are subject to auditing.
7. Acceptable use
You may not submit false, automated, excessive, misleading, harassing, or unauthorized records; impersonate another person; probe or bypass security; access another organization’s information; upload malicious code; interfere with service availability; use business introductions for indiscriminate solicitation; or use the service unlawfully.
The platform may use rate limits, duplicate controls, anti-bot technology, verification, audit trails, and suspension to protect patients, customers, and the service.
8. Privacy
Information handling is described in the Privacy Center. A participating organization’s HIPAA Notice of Privacy Practices or other notice may apply after information is transmitted to that organization. Privacy rights may be exercised through Your Privacy Rights.
9. Availability, third parties, and changes
The service may depend on hosting, security, verification, communications, scheduling, and other providers. Cardiology Insights does not guarantee uninterrupted availability or the performance of an external service. Features, clinical releases, routing configuration, notices, and these Terms may be changed to improve safety, security, compliance, or operations.
10. Disclaimers, responsibility, and legal terms
To the fullest extent permitted by applicable law, the public service is provided on an “as available” basis without a guarantee of a particular medical, scheduling, payer, or business outcome. Nothing in these Terms excludes responsibility that cannot lawfully be excluded.
Customer agreements may contain additional warranties, indemnities, liability limits, governing law, venue, dispute procedures, and service commitments. For public users, applicable mandatory consumer and privacy protections remain in effect.
