1. Acceptance and eligibility
By using this service, you agree to these Terms of Use and acknowledge the linked Privacy Notice. The service is intended for adults. Do not submit another person’s information unless you are legally authorized to do so.
These terms apply to the technology service. Additional terms, financial policies, consent forms, and privacy notices of Heart & Vascular Partners may apply if you become a patient or schedule an appointment.
2. Educational and clinical-support limitations
The “I Know My Numbers” calculator and adaptive symptom tool provide educational risk information, screening support, urgency guidance, and operational routing. They do not establish a definitive diagnosis, prescribe treatment, confirm that a condition is present or absent, or replace evaluation by a qualified healthcare professional.
Results depend on the information entered and on the versioned calculation, clinical knowledge base, safety rules, and routing configuration in use at the time. Incomplete, inaccurate, changing, or misunderstood information can affect the result.
3. Emergency care
Do not use this website to obtain emergency care. Call 911 or the applicable emergency number for severe or ongoing chest pressure, severe shortness of breath, fainting, stroke symptoms, sudden severe pain, or other symptoms that may be dangerous.
Emergency safety instructions take priority over ranked results, contact verification, insurance matching, location preferences, and routine assisted scheduling. A patient who receives emergency instructions should not wait for a callback or appointment request.
4. Adaptive assessment and clinical governance
The adaptive symptom pathway may update a ranked differential after each response and select follow-up questions using a versioned clinical-support engine. Safety rules operate separately from ranking and may stop routine routing.
Clinical knowledge-base releases are controlled by Cardiology Insights clinical governance. Clinician feedback does not automatically modify the live engine. Proposed changes are reviewed, versioned, regression-tested, and approved before publication.
The platform may recommend a type of care or specialty, but the participating medical practice makes the final clinical, physician, location, and scheduling decisions.
5. Routing and assisted scheduling
A verified request may create a Patient Access Command Center task for Heart & Vascular Partners. The platform may recommend a specialty, physician, location, urgency, and access pathway using current customer configuration.
A route, task, contact request, verification code, or displayed recommendation is not a confirmed appointment. Insurance participation, referral requirements, physician availability, location capacity, appointment type, and appointment time must be confirmed by the participating organization.
Use of this service does not by itself create a physician-patient relationship with Cardiology Insights or with a listed physician. A clinical relationship is established only under the participating practice’s applicable procedures.
6. Contact authorization, verification, and communications
When you submit the assisted-scheduling form, you authorize Cardiology Insights to transmit your assessment, urgency, routing, insurance-selection, and contact information to Heart & Vascular Partners and authorize Cardiology Insights and the participating organization to contact you about verification, routing, scheduling, and this request.
Verification messages and patient-access communications are transactional communications related to the request, not consent to unrelated marketing. Standard carrier or data charges may apply to text messages. You are responsible for providing contact information that belongs to you or that you are authorized to use.
7. Acceptable use and abuse prevention
You agree not to submit false, automated, excessive, misleading, harassing, or unauthorized records; interfere with the service; attempt to access another organization’s data; impersonate another person; or use the service for unlawful purposes.
The service may use rate limits, duplicate detection, a hidden bot field, completion-time checks, Cloudflare Turnstile, and one-time verification. Suspicious records may be blocked, consolidated, or placed into a separate review queue. An unverified submission does not create an ordinary scheduling task.
8. Privacy and multi-company separation
Use of personal and health-related information is described in the Privacy Notice. Customer organizations operate in separate tenant environments. Customer users are authorized only for their assigned organization and permitted location or role scope.
Depending on the deployment and contractual relationship, a participating medical practice’s HIPAA Notice of Privacy Practices and other legal requirements may apply after information is transmitted to that practice.
9. Third-party services and external links
The service may use or link to third-party verification, security, communications, hosting, scheduling, mapping, or practice-management services. Third-party services are governed by their own terms and privacy practices. Cardiology Insights does not guarantee the availability or performance of an external scheduling or communications service.
10. Changes, suspension, and contact
Cardiology Insights may update the service, these terms, clinical-support releases, routing configuration, verification controls, or available features. Material changes to these terms will be posted with a new version and effective date.
Access may be limited or suspended to protect patients, customer organizations, system security, or service availability, or in response to misuse or legal requirements.
