Still short of breath or exhausted despite heart medications?

This short questionnaire helps you (or a caregiver) summarize key signals commonly used in advanced heart failure evaluation. If it looks promising, you can send a one‑page “Signal Packet” to request next steps.

Standard medications help manage heart failure, but they don’t always address heart strength. If you’re still symptomatic, you may be a candidate for heart‑strengthening technologies (CCM) currently in advanced evaluation.

Emergency warning: If you have chest pain, severe shortness of breath, fainting, or stroke symptoms, seek emergency care immediately.

What we ask

Simple, high‑signal items: daily activity limits (NYHA‑style), whether meds are helping, known EF (LVEF) if you have it, and basic device history.

What you get

A clear result (high‑conviction match / possible match / not a match today) plus a printable “Signal Packet” you can share with a participating clinic or keep for your cardiologist.
