This policy describes how Cardiology Insights (“we,” “us,” “our”) collects, uses, and shares information when you visit
cardiology-insights.com and use our educational cardiovascular screening tools.
Important: This website is not emergency care and does not provide medical advice. We are not a hospital, insurer,
or health plan, and this website is generally not governed by HIPAA.

- Risk calculations are performed in your browser. We do not automatically store calculator entries on our server unless you choose to submit information (for example, to request a voucher).

Notice at collection (California)
If you are a California resident, this section provides notice of what we collect and why, at or before the point of collection.

- Information you provide in free text (notes you type into a form).
- Screening context you submit (for example, symptom selections count, risk band, or other context passed with a voucher request).
- Sensitive personal information may be included if you choose to provide health-related details in notes or screening context.
Purposes for collection and use
- Provide the website and tools you request (including generating and delivering a lab voucher, if offered).
- Respond to your requests and coordinate next steps you ask for (e.g., voucher delivery, routing to nearby options).
- Protect the security and integrity of our site (fraud/spam prevention, debugging, abuse monitoring).
- Comply with applicable law and enforce our terms.
Sale/sharing
We do not sell personal information and we do not share personal information for cross-context behavioral advertising.
Retention
- Voucher/contact submissions: typically retained up to 24 months after your last interaction, unless a longer period is required for security, dispute resolution, or legal compliance.
- Security logs: typically retained up to 90 days, unless needed longer to investigate abuse or incidents.
We do not retain personal information for longer than reasonably necessary for the purposes described above.

2) Information collected automatically
Like most websites, we may automatically collect basic technical information to operate the site and prevent abuse.
- IP address and user-agent (browser/device information)
- Pages visited and approximate timestamps
- Security and error logs
3) Cookies & local storage
Our tools may use browser storage (for example, sessionStorage) to carry your screening context between pages.
This storage is used for functionality and does not, by itself, identify you.
How we use information
- To provide services you request (voucher delivery, communications you opt into, and coordination of next steps).
- To operate and improve the site (debugging, quality assurance, and performance monitoring).
- To protect users and the service (fraud prevention, spam filtering, and security incident detection).
- To comply with law and enforce our terms.
We do not use your information to make automated decisions with legal or similarly significant effects.
How we share information
We may share information in the following limited situations:
- Service providers that help us host and operate the site (e.g., web hosting, email delivery), under contracts that restrict their use of data.

- Right to delete personal information we collected from you (subject to legal exceptions).
- Right to correct inaccurate personal information.
- Right to opt out of “sale” or “sharing” of personal information (we do not sell or share for cross-context behavioral advertising).
- Right to limit the use and disclosure of sensitive personal information (we use sensitive information only as necessary to provide the services you request and for security/compliance).
- Right to data portability where applicable.
How to submit a request
We may need to verify your identity and request additional information (only as needed) to protect your account and prevent fraud.
You may use an authorized agent where permitted by law.
Global Privacy Control (GPC)
If we ever engage in “sale” or “sharing” as defined by California law, we will treat valid browser-enabled Global Privacy Control (GPC)
signals as an opt-out preference where applicable.
Do Not Track (CalOPPA)
Some browsers include a “Do Not Track” (DNT) signal. Because there is no consistent industry standard for responding to DNT,
our site does not respond to DNT signals.
We do not currently allow third-party tracking for cross-site behavioral advertising.
Children’s privacy
This site is intended for adults. We do not knowingly collect personal information from children under 13.
If you believe a child has provided us information, contact us and we will take appropriate steps to delete it.
